(Possibly) reverse the strap!
This device used to be called a truss in my father's time. He, too, had a hernia (inguinal) in his lifetime. Apparently, sometimes runs in families. Anyway, this is a pretty good and well-built item. Copious velcro is used to adjust and secure the belt(s). There is a hip-belt and a tightening crotch belt. The waist belt must be installed high on the hip opposite the (inguinal) hernia and must be tight(!) to prevent slipping down while walking, etc.It would have gotten 5 stars had it been properly assembled at the factory. The strap that adjusts (tightens) the hernia pad was installed backwards. This made it very hard to properly tighten the strap and required twisting the strap, making it less than comfortable as the buckle for this belt ends up between your legs (wear the device OVER underwear to reduce discomfort from possible chaffing). Once I figured this out, was much easier to put on and adjust. I would order again.

Surprising relief from hernia belt.
While waiting 3 weeks for my hernia repair surgery, this belt allowed me to keep up all my activities without pain. The constant pressure it provided allowed me to hike in the mountains, walk the dog, ride my bike, and work in the yard. It is a relief to take it off at the end of the day, but the freedom it provided was worth the minor discomfort of wearing the belt.

Good quality but not a good option for biking (road or stationary)
Upon opening the package, I liked the product. It looks well-made and the design makes sense.I tried it on and got it to a fairly comfortable fit. Now I'm able to stand and walk and do most normal activities without fear of my inguinal hernia popping out.The inner pocket is a nice touch - it allows me to put custom padding without it moving around. The included pad is a bit thick for me but it's usable. I don't understand why people complain about that - you don't HAVE to use it. Socks and various pieces of fabric can be stuck in there for a truly custom fit.I would have given this 5 stars but read below for why I dropped it down to 3.THE BAD:My main complaint about the design is the plastic buckle that you loop the groin band around. It sticks out beyond the fabric so when things get tight, it will dig into your nether regions and the area where your thigh meets your groin. Because of this, it can be hard to get comfortable when sitting on a couch.I bought this so I could resume my Peloton bike workouts. If you are wearing padded bike shorts, this will definitely dig into your groin area. Very uncomfortable and downright painful. I tried messing around with positioning but couldn't tolerate it for more than 5 minutes. I tried riding with plain athletic boxer briefs and could get them to work but I lose the padding protection for my butt.I would return this if I could but I already used it. I wish they could have positioned the plastic buckle half an inch higher - that way it is shielded by the fabric.Bottom line:If you want to use this for bike riding, look for something without a plastic buckle sticking out.

Has best crotch strap
Fit and finish is fine. Big pad does the job. I bought a 2nd truss (cannot wear same one ALLLL the time) What bears the bell away is the crotch strap. It has a loop that goes on the belt (in back) and a solid Velcro strap adjustment that uses the strap itself. Other trusses that only wrap the strap around the belt in back are flimsy, likewise when the strap is only attached in front to Velcro on the front of the pad it comes loose gets cheesy. EMO does not show the loop attachment in the description for some reason unknown

If you want to lift safely with a hernia get this.
This belt allows me to work out with my inguinal hernia. Gives the support and therefore the confidence to progress without making things worse.
