Yes, it depends on which way you turn it. It will count downwards if you turn it backwards though.

What would be the recommended way of adding a wire tension interface to this unit?

Hello! Do you mean a way to prevent loss of tension in the feed-wire if winding is interrupted? If so, I'm not sure that there is a 'recommended' way. However, you could try This: Fit a cylindrical axle (even a pencil would suffice) through the bore of the spool from which wire is being removed. Glue the pencil to the spool, so that both must rotate together. Orientate the pencil horizontally and put a stout rubber band over it, so that the band hangs vertically downwards. Lash-up a couple of rudimentary bearings, arranged to support the pencil horizontally. Drop the horizontal pencil into them. Now attach a weight to the bottom of the rubber band. The weight must be hanging freely and putting some tension into the band. If you get the weight right, when you wind, tension on the wire between growing coil and feeding spool will overcome the pencil-to-band friction. So, the wire will be pulled from the spool onto the coil. When you stop winding, the band/weight combination will prevent the spool from turning, thereby preventing loss of wire tension. It would need some trial-and-error to get everything right. In fact, I'm going to try it myself. Hope that this helps.

Can i use this to wind or unwind mainsprings of clocks

I have never tried (or even considered) this use for it, but my gut feeling, based on my use of it, is no. It seems to me to be most appropriate for wire gauge materials, albeit quite thick wire. I suspect that the flat steel in a mainspring would not be suitable. But this is only based on my gut feeling, I'm afraid.

Can the chuck be turned manually both clock and anti clockwise and does not spring back or forward when you release the handle?

Yes you can use it in both directions
