I suffer from frequent migraines, and this ice cap has been an absolute game-changer. Good fit even for large head like mine. Freeze or ice pack 10 minutes, wear 10-20 minutes, repeat.
I have a large head, I was expecting this to not fit, but it has plenty of stretch. I tried putting this in the freezer for 2 hours, I put it in a resealable bag so it didn't pick up freezer smells, it got so cold even in warm weather it was uncomfortably cold to wear longer than 10 minutes. Even after overnight in the freezer this stayed flexible. The first couple minutes putting it on though, very refreshing, especially coming out of hot summer outside weather. I also had a little hot weather sinus congestion, it cleared almost immediately after putting this on, but closer to 10 minutes I started getting cold sinus congestion from this probably still being around 60F or cooler. I can see why the instructions say 10 minutes in the fridge as the minimum, it's probably all you need to wear it cold. If your room is cooler than body temperature (96F) this compress will already feel cool to the touch, but cooling it via room temperature is probably impractically slow if you want to use it for extended time. If you don't want to walk to the fridge, I suggest keeping a freshly frozen ice pack in a tray near you, and between wearing the compress, sticking the ice pack inside the compress for maybe 5 minutes, it should provide plenty cooling for 10-20 minutes until your ice pack isn't that cold anymore (which could be up to 2 hours from taking the ice pack out of the freezer if you have a large enough ice pack).If you're still unsure about this, here are some alternatives to try, they may be good enough for you as is: bag of ice definitely simple but hard to arrange so you get the cooling all over your head; wet towel from the fridge might drip and will get your pillow damp; ice pack to cool your pillow might get the pillow damp and the cooling won't last as long. If you just want to cool your head off outside, my current favorite way is to use a spray bottle of plain water, spray my head, and have a breeze or portable fan, optionally wearing a water-soaked quick-dry hat.

I get brutal tension headaches and finally caved on buying this. I was skeptical, but it's honestly amazing. I keep it in the freezer and when a migraine starts, the cooling relief is instant.
