When light strip fails is a whole new light fixture required or can replacement light strips be purchased and if so how difficult to replace?

You have to replace the fixture. But it has an expected life of 50,000 hours. So the real question is how old are you?

do they mount to a standard 4" octagon fixture box?

Sorry, I am not sure. They are set up for a variety of standard boxes so they probably would. I always mount a 4' fixture with the holes near the ends, just a personal preference.

Do these actually draw 40 watts, or are they 40 watt equivilant ? example: some led lights are 25 watt equivilant but only use 10 watts?

They draw 40 Watts, (.33 amps ) 4000 lumens each.

Is the lens polycarbon or easy brake plastic?

The ones I received were a semi thick plastic, and didn't break upon installation... Actually had no issues with them other than the shipping..
