Video shows screws being removed from switch plate and then screwed back in off to the right. Do I have to make new holes in my switch plate???

No new holes ! Simply screw on top is switch plate. Nice idea my son's father in law kept shutting the boiler off instead of the basement lights

Will this device fit over a standard wall outlet, preventing things from being plugged into the outlet?

This is designed to fit over a rocker switch. The mounting design for Deco-style and GFCI outlets is the same as a rocker switch. It will fit over Deco and GFCI outlets to prevent things from being plugged in. It will not fit over conventional duplex style outlets due to a different mounting design.

What is the size of the hole in the center of the cover?

the open space inside the lock out cover is 1 1/2"H x 1 3/8"w this will directly go over a standard toggle switch with cover, or a rocker switch with it's cover. if you remove the tabs inside you will have a open space of 3" H x 1 3/8" w, but when doing this the cover has to be mounted to the mounting screws of the switch. we will add additional pictures to our listing to help you
