I build thick medium size tables that have end grain leg tenons through the top, is this the best option, if not please direct me accordingly

I think a block plane is better for end grain. The other is a great plane for most everything. It just long and heavy to balance on the end of a tenon.

Does this plane come with set up instructions?

not really, you should view rob cosman videos on how to prepare a plane for service, sharpening, setting and using. As far as the plane is concerned, it is an excellent plane I just wish I went up to a 4 or 4 1/2 size but money was tight and the plane still gets the job done.

Is this a USA made tool?

China. Quality is excellent. You do have to "finish sharpening" the iron a bit to improve performance.
