Is this TSA approved?

The TSA does not approve or disapprove of medicine carrying devices. You're allowed to bring with you any amount of liquid medication necessary in any container you'd like including frozen or partially frozen ice packs. I've traveled with just regular ice pack and a cooler to hold my insulin. It also does not count against your carry-on item

Can I put my pen in this too? Or is it just for the vials?

Where can I buy a replacement ice pack? Just in case something happens to it.

Yes .... All of those ice packs are similar size I have one from a previous pack from a different company and they all fit
