is the water hose connection on the outside metal or plastic.

The connection is plastic. Units with plastic connections have a 3 year warranty. I have had mine for 6 years and it works and looks fine. Apply pipe thread tape and tighten the connections with a slip wrench. I only have a 25 ft rubber hose on the reel in my patio and it works great.

Is there a piece in the back where you connect the garden hose or is this just to store it away?

There is a piece to attach to the hose and to the spout from the house/water source. There are also stakes to sink into the ground to keep it in place while winding.

Does this container handle operate a level wind slide for cable storage?

No. The pictured slide isn't connected to the handle in anyway. It freely slides back and forth and is nearly useless. I should have researched this more before buying it.

This would sit on concrete near the water spigot. Description says it has stakes to anchor to ground. Are the stakes removable?

Stakes is a really generous term for the pins that come with the unit. They are removable for use on concrete. HOWEVER...be prepared for some aggravation on concrete. This unit is light weight and slides very easily if you tug on the hose at all. You have to hold onto the unit and unwind as much hose length as you need for the job before you start.
