The four types listed are carbon dioxide, dry (cartridge type), dry (stored pressure type), and pressurized water. The blank grid has four columns with headers that read "Date," "By," "Date," and "By," and continues on the back of the tag. A note on the front reads, "Attach this tag to fire extinguisher. Recharging and inspection records are to be entered by inspectors" to help describe the purpose of the tag. It includes a 3/8" diameter hole for fastening. The tag measures 6 x 3 inches (H x W). (H is height, the vertical distance from lowest to highest point; W is width, the horizontal distance from left to right.)
