Q: Do these wrenches have a grove cut out inside the square drive hole so as to be heald by an extention bar?

Not quite sure what your question is in regard to, but no they just have a square hole like on any socket. Anything you can stick into a socket (a ratchet, an extension etc) will fit into them. I used a 3/8" extension on the size I needed and the wrench did not fall off if that's your question.

Q: Using them for brake lines, how well does a 3/8 ratchet clear the opening? Do you have to use a 3/8 breaker bar with them?

Did not use on brake lines. I used them on power steering lines

Q: Are they offset ? If you flip them over can you get a different bite on the fastener

They are offset but clearance around nut or bolt may inhibit how far you are able to rotate with each bite. I changed out a power steering high pressure hose with mine. It was tedious but did the job.
