It has 5 holes, and 4 plugs. Basically they assume you will use at least one.

How can it be 4.2 inches by 5.5 inches? That's not a circle. So, is it safe to assume that the 5.5 inch measurement includes the mounting tabs?

Yes ,that point would be the widest footprint when mounting this box 5.5" . That means that the space that this box would fit between two fixed objects is 5.5"
