New CMS 1500 Claim Forms - 50 Sheets (02/12 Version) for Laser or Inkjet Printers

I needed a few of these new HCFA1500 forms to test programming changes and did not want to buy packs of 100 or 1000. These were perfect. They were inexpensive and came quickly in the mail and are the correct form. If I need more, I will certainly buy them again.

Forms are fine - what is there to say about forms for Medicare. I just love them. I hope I will need more.
