Do you need to pull the shelf up with your hand? If so, how much strength/ leverage do I need?

Yes, you need to pull shelf up with two hands and it lock will lock in place. When you want it to collapse u push release levers and support it back into cabinet. It is not difficult.

If it is under a drawer, can it still raise to counter height?

No it cannot not unless you remove the drawer and reconstruct without it. I am returning my shelf for that reason.

Does this include the shelf shown in the picture? Also, why is there no product description of this item?

Did not include the shelf part was only the brackets and I ended up returning

Can this unit be installed below a drawer?

Yes I had my handyman install and it is under a drawer
