Is this a flat tape or string floss?

It's Flat; the only kind I buy, 'cause the string breaks and gets stuck in teeth. This product doesn't break under normal use.

Does this have pfas in it

Yes. There are other flosses composed of dakron & wax (traditional method) that do not contain PFAS. However, traditional floss is not as slippery as flosses containing PFAS, & as a result consumers are less likely to use it.
