It stays cold enough that you're not over doing the cold therapy. So that's a good thing if you fall asleep with it on you're not overdoing the chill. END OF UPDATE/I have some kind of tendonitis, bursitis or 9something in the upper outside portion of my arm that is extremely painful. It's also mostly immobile. This is a one size fits all item. A little confusing on how to get the whole thing situated. Turns out that it really wasn't that difficult, the confusing came on my part. The ice pack that comes with it gets really cold, and it will give your shoulder and upper arm a good chill. I've only ice packed it once, there is a little bit ease of pain. Then again I really don't use it much these days. I'm also using it as a support because I just had knee replacement surgery and I'm sleeping in a recliner. One night the pain woke me up because my shoulder had dropped to the side. I'm hoping that the more I treat it with the ice pack the better it'll get.
